Tuberculosis Housing and Directly Observed Therapy

The program serves individuals with active tuberculosis who are homeless at the time of discharge from the hospital. A case manager dedicated to the project is responsible for moving participants into program housing, securing medications and related supplies, providing daily observation to ensure adherence to medications, ensuring continuity of care for tuberculosis and other health conditions at Heartland Alliance Health Centers, linkage to community resources, and coordination with the Chicago Department of Public Health through monthly reports, case reviews, and as other areas as needed.
