Heartland Alliance Health (HAH) is a federally qualified health center with clinics located in Uptown, Englewood, and the West Loop. More than 75 percent of our participants are homeless, many living with multiple chronic mental and physical illnesses. HAH also delivers care at shelters and community settings through medical outreach teams.

HAH provides participants with preventative care, urgent care for acute illness and on-going primary care and disease management for chronic conditions.

The health centers provide comprehensive primary care services, including:

• Family Medicine
• HIV Specialty Care
• Medical Case Management
• Prenatal Care
• Medical Interpretation
• Internal Medicine
• Benefits and Entitlement Assistance
• Laboratory and Prescription Services
• Medication Assistance
• Referrals to Specialists

HAH is a level three certified Patient Center Medical Home. All services are provided based on participant need and choice with healthcare professionals partnering with participants to improve their health. HAH provides comprehensive, integrated care, coordinating health center services with housing, nutrition, mental health, addictions and case management services offered by other departments to holistically meet participants’ needs.

Six days a week, participants can schedule an appointment at the health centers. Walk-ins are welcome, but are seen based on availability.

Each year, our multicultural, multi-lingual team serves more than 8,000 people, helping them improve their health and rebuild their lives.

We serve specialty populations, including:

• People experiencing homelessness
• Refugees
• People with HIV
• People with serious mental illness

Participants may also sign up for the participant portal to communicate electronically with the health center staff and view their health records.
