Heartland Alliance Health’s residential housing programs, Antonia Permanent Housing and Safe Haven and Pathways Home, serve people experiencing homelessness who are struggling with co-occurring mental illness and substance use issues.
On-site staff support is provided for 90 single-room-occupancy units. Forty-six units are staffed 24 hours a day, and 44 units are staffed 12-14 hours a day.
Located in Uptown, the programs offer residents a safe environment where they can receive supportive services, build skills and plan their path to recovery.
Our approach is based on the philosophy of harm reduction, helping participants to identify and decrease their high-risk behaviors and increase healthy behaviors. Abstinence from alcohol and other drugs is not required in order to be eligible for housing and services.
The outcomes for residential services confirm their effectiveness: 97 percent of residents adhere to their psychiatric medications and 100 percent of those recently discharged remained housed 60 days later.
Community Housing
It’s hard to imagine being able to get healthy without having a safe, stable place to call home. Heartland Alliance Health believes that housing is healthcare, the very foundation that allows homeless individuals and families to engage in medical services and consistently take their medications.
HAH leases more than 350 apartments across the city, providing supportive housing to participants who are homeless or on the verge of homelessness. Housing services target people who are homeless and severely mentally ill or addicted, individuals with chronic illnesses, including HIV, and people in treatment for tuberculosis.
Participants are expected to pay 30 percent of their income in rent. HAH’s community support teams and case managers assist participants in locating an appropriate apartment, moving in, getting the furnishings needed and building the skills required to successfully live in the community. Each year, HAH provides more than 50,000 nights of safe, stable housing to more than 300 participants.
