Finding Home in a Pandemic

Heartland Alliance Health connects thousands of our most vulnerable neighbors to the critical care they need through our network of health centers, food pantries, and homeless outreach services. These last few months, clinicians and social workers from HAH have further integrated their systems of care to increase access to healthcare, housing, and wrap around services for our participants – all while facing COVID-19 – and the outcomes are life changing.
