Team Tahoe

Team Tahoe is an integrated healthcare team that serves participants who are high users of medical services; this team is intended for people with chronic medical conditions as well as behavioral health issues, particularly mental health or addiction. The team is comprised of two medical providers, along with a therapist, a mental health clinical practitioner who provides case management, and an Alcohol and Other Drug Counselor.
