West Cook Housing Action Network for Choice Endeavors and West Cook Housing Initiative Partnership are scattered-site supportive permanent housing programs located in West Suburban Cook County. They serve individuals who are HIV positive or living with AIDS and who may have other chronic health conditions, including mental illness or substance use disorders. Residents receive case management that focuses on achieving medical stability and, when indicated, mental health conditions.
