Pathways Home is a continuum of residential and outpatient services designed to address the needs of homeless adults with serious mental illness and co-occurring substance use disorders. Pathways Home is made up of three component programs: Safe Haven, Permanent Housing, and the outpatient program.
The Pathways Home Safe Haven is a community housing program designed for participants who find participation in treatment for their mental illness and substance use disorder particularly difficult.
Pathways Home Permanent Housing is a community housing program designed for participants that are ready to look, learn, and talk about their illness, even if they have not yet attained consistent psychiatric stability or consistent management of their substance use.
