AFC/Safe Start project (formerly known as Medicaid Supportive Housing) targets people who are homeless and have a disabling physical condition, a mental illness or substance use disorder and are high users of Medicaid. Supportive services are designed to coordinate medical in order to improve health, reduce emergency care and optimize stability in the community. Individuals who are eligible for this program must be eighteen or older, homeless, have a physical condition and are identified as high users of Medicaid.
